Target group
This course is targeted to adults social care staff and those in residential care, nursing care, home care and supported living settings, personal assistants and other frontline practitioners who have regular contact with service users/patients and their families or carers in the London Borough of Sutton.
Staff Groups A, B, C, D, and E as per the Bournemouth University National Mental Capacity Act Competency Framework.
Level 2 staff and above (ref: NHS Intercollegiate document 2018 - Adult Safeguarding: Roles and Competencies For Healthcare Staff).
Aim
The aim of this event is to explore what good practice looks like in the context of Safe Discharge Planning from Hospital to Home. Home is defined as accommodation whether this be one’s own home, supported living or a care home setting.
Discharge from hospital tools will be launched at this session and this includes both a discharge checklist and grab sheet. The multi-agency discussion will enable partners to discuss the feasibility and inform them of the ease of use to ensure that these documents will be utilised effectively amongst partner organisations.
Learning objectives
After attending this event, participants will:
- Have increased their knowledge and understanding about the impact of different parts of the system, and what is needed to achieve safe discharge
- Have explored the toolkit and be better equipped to think about when and how it could be used
- Have developed an understanding about the importance of communication and flow of information to provide evidence of safe discharge from hospital
