Discharge from hospital tools will be launched at this session and this includes both a discharge checklist and grab sheet. The multi-agency discussion will enable partners to discuss the feasibility and inform them of the ease of use to ensure that these documents will be utilised effectively amongst partner organisations.

After attending this event, participants will:

  • Have increased their knowledge and understanding about the impact of different parts of the system, and what is needed to achieve safe discharge
